This website has been constructed by the Lordswood PPG Working Group. This team is made up from volunteers from the Lordswood PPG.
This privacy notice explains how Lordswood Medical Practice PPG collects, manages, uses and protects your data. It outlines the type of data we hold and how we use it to interact with you.
The categories of information that we process include:
· personal identifiers and contacts (such as name and contact details)
Why we collect and use information
We collect and use your information, for the following purposes:
a) to communicate with you regarding the activities of the PPG
b) to respond to a query, which you have initiated with the PPG
Under the General Data Protection Regulation (GDPR), the lawful basis we rely on for processing information is:
· legitimate interest, necessary for the function of the PPG
How we collect your information
The information collected is given by you on a purely voluntary basis, when you initiate contact with the PPG via email. If you express an interest in joining the PPG, further information regarding name and contact details will be collected. No information is collected via Lordswood Medical Practice.
How we store information
We hold your information securely for the period of time you are part of the PPG, within Gmail. Only two members of the PPG who have undergone GDPR training have access to the information held within Gmail. Any information sent to us involving further sensitive personal data, such as health conditions, will be immediately deleted. This information will not be disclosed without your explicit consent, as detailed below.
Who we share information with
We do not routinely share your information with anyone. The only circumstances in which we would pass on your information is if you volunteer to attend a meeting or join a committee as a representative of the PPG. In this circumstance we would seek your explicit consent to pass on the contact details necessary to the organisation to communicate with you.
Requesting access to your personal data
Under data protection legislation, you have the right to request access to your personal information we hold. To make a request for your personal information contact the PPG secretary. You also have the right to erasure of your personal information, which can be requested via the same soute.

The Lordswood PPG exists to support the Lordswood Medical Group by representing the views and opinions of patients. We work closely with the Practice, but we are independent and the Practice is not responsible for the content of this website and any errors it may contain.
